    Research the literature to determine medicare requirements for interventions for Diabetes and also best practices to be incorporated into careplan. Meet with mentor to review proposal and to get input for project. Meet with clinical managers to educate on process and to get their input. Meet with clerical staff who distribute chart reviews to educateon process. Fine tune proposal. Read Chapter 9 in Billings & Halstead to get help with writing outcomes. Review Bloom;s taxonomy.
  • Chart reviews

    Admission and discharge chart review for 2 weeks to determine clinicians who need education. Admission reviews will reveal plans of care without interventions and discharge reviews will show those charts without documentation on the measures. I will develop a simple tool to collect data. SHP reports will be monitored daily. Development of educational plan over this same 2 week period to inlude research on effective small group teaching.

    Chart review to include looking for interventions for meds, blood sugar monitoring, footcare, and other best practices found in the literature.
  • Class work

    Teach class to those clinicians who have the opportunity to improve documentation

    Class instruction

    Samm group instruction with those staff showing a need for improvement. Considerations for selection will include proximity to main office and patient schedules.
  • Start post education chart reviews

    These chart reviews will be on the staff who received the education to evaluate if they met their learning outcomes and if teachng was effective.

    Program evaluation

    Place data in an excel spreadsheet. Compare data from pre and post education. Evaluate over all SHP score for time period involved.Work on abstract and paper and power point for presentation.
