Starting with a complicated pregnancy, baby Sunny was born at 39 5/7 weeks gestation. The pregnancy was complicated by polyhydramnios and only two vessels in the placenta cord.

Baby Sunny was diagnosed with chromosome 7 microduplication, tracheoesophageal fistulas, type 2 laryngeal cleft, atrial septal defect (ASD), ventricular septal defect (VSD), and patent ductus arteriosus (PDA).

Baby Sunny needed a surgery to close the fistula between her trachea and esophagus. With this tunnel connecting the two organs, Sunny's ability to eat and breathe are extremely impaired and dangerous.

Because there is a defect in the septal wall between both the atria and ventricles, Baby Sunny must have surgery to close the holes. A patch is used to prevent back flow between atria and ventricles.

Nutrition is poor due to the recovery from the tracheoesophageal fistula. Baby Sunny has a G/J tube placed to supply total enteral nutrition as well as administer medications.

Baby Sunny developed a second tracheoesophageal fistula that must be repaired. The connection is again closed between the trachea and esophagus.

Baby Sunny needed this surgery to repair the laryngeal cleft palate that she was born with. This surgery recontructed the larynx to remove the cleft.

This is the final tracheoesophageal fistula repair for Baby Sunny. The fistula was repaired using a slide tracheoplasty, which essentially prevents more fistulas to form.

During the final repair of the tracheoesophageal fistula, Baby Sunny had a patent ductus arteriosus (PDA) ligation. This surgery closes off the connection between the aorta and pulmonary artery using sutures at each end to tie it off.

Because the VSD and ASD opened, they again closed it with patches.

Because the father is unavailable to help with cares, it is necessary to designate a second person as a care provider. The mother's younger sister has agreed to learn the different cares associated with a trach/home ventilator and to become the second care provider.

Baby Sunny had a tracheostomy placed to aid in breathing and to set up a home ventilator. She will go home soon with the home vent.

The landlord will not comply with the electrical updates needed to support the home ventilator. Mother may have to look for a new apartment.

Team will hold a discharge meeting with mom to go over independent cares and determine any extra teaching needs before discharge.

Mother will sign lease today and move in on Oct. 1. This new apartment will support the elecrtical needs of the home ventilator.
