    NC Piedmont Behavioral Health

    This waiver gets its authority from the 1915(b)(c) waiver. The purpose of the b portion of this waiver was to allow selective contracting to provide mental health, developmental disability, and substance abuse services to all age groups in Cabarrus, Davidson, Rowan, Stanley, and Union counties.

    NC Be Smart

    This demonstration gets its authority from section 1115 of the SSA. It provides family planning and family-planning related services to men and women over the age of 18 who have a family income at or below 185% of the federal poverty level, and who are otherwise not eligible for Medicaid. This demonstration was approved in North Carolina on November 5, 2004, made effective on October 1, 2005, and expired January 31, 2013.

    NC CAP Choice

    This program gets its authority from the 1915(c) waiver. Its purpose is to provide adults with day health, institutional respite, in-home aide, a personal assistant, a care advisor, the Financial Management Services, consumer-directed goods and services, home mods and mobility aids, meal preparation and delivery, in-home respite, and telephone alerts. This waiver applies to individuals aged 65+ and disabled individuals aged 18-64.

    This program gets its authority from the 1915(c) waiver. Its purpose is to provide adults with day health, case management, institutional respite, a care advisor (CHOICE option only), assistive technology, home mods and mobility aids, meal preparation and delivery, non-institutional respite, participant goods and services, a personal assistant (CHOICE option only), a personal care aide, the Public Employee Retirement System, training and education, and transition.

    Provides individuals personal care services such as in-home respite care, case management, nursing services, pediatric nurse aide in-home respite care, pediatric nurse aide services, caregiver training and education, community transition funding, home mods, in-home nurse respite, institutional respite, motor vehicle mods, palliative care-counseling, and palliative care-expressing therapies. The waiver also supplies an adaptive tricycle.

    NC Supports Waiver

    Its provides adults with day health, day supports, home and community supports, personal care, respite, behavior consultation, crisis respite, crisis services, the Public Employee Retirement System, home mods, individual goods and services (SD only), long term vocational supports, specialized consultative services, specialized equipment and supplies, and transportation. It also provides supported employment and teaches individuals and caregivers proper training.

    This waiver gets its authority from the 1915(c) waiver. Its purpose is to provide individuals with day health, day supports, personal care, residential supports, respite, supported employment, a behavior consultant, crisis respite, crisis services, home and community supports, home mods, individual goods and services (SD only), specialized consultative services, long term vocational supports, specialized equipment and supplies, and transportation. This waiver also provides help with transition.
