Changes in residency education based on competencies and improved assessments in performance and identifying outcomes to improve education. These changes would improve outcome measures for accreditation.

Challenges in implementing competencies for both curriculum development and assessment and the lack of a shared mental model resulted in the development of milestones. ACGME developed narratives that described educational trajectories through residency education.

The first milestones were implemented in 7 core specialties (emergency medicine, internal medicine, neurological surgery, orthopedic surgery, pediatrics, diagnostic radiology, and urology)

The remaining core disciplines and majority of subspecialties implemented their milestones.

Iterative reflection of the implementation of the milestones identified limitations of the original set of milestones. This reflection resulted in a revision which harmonized elements and simplified the complexity and increased the community engagement.
