Mia's family is driving to visit her dad's parents and a good friend of the family, who just had a baby, when their car gets hit by a drunk driver. All four of them are thrown from the car. Her parents die instantly and her brother, Teddy, dies later that day in the ER. Mia is put into a comma from impact and is in bad shape. She is rushed to a hospital and airvacked to a larger hospital to deal with her wounds. She has many serguries in the afternoon and evening of this day.

In "If I Stay" it is unclear how long Mia is in a comma. In the sequal, "Where She Went", it says it lasted 3 days. Many people came to visit her, including Adam, Kim, many of her dad's realitives, and her mother's best friend and family. Adam was taking to Mia and telling her how much he needed her when she woke up.
