The patient's grandpa on her moms side was born on July 5th, 1948. He lives with her grandma and they very close. Grandpa worked in retail and at a factory and is now retired. He and Grandma are the main support people in the family. -
Grandma
The patient's grandma on her moms side was born on 10/27/51. They are very close and live 5 minutes apart. Grandma was a unit case manager and is now retired. -
Mom
Mom was born on February 22nd, 1979. She has 3 daughters and is not married. She has lived in Milwaukee her whole life and does not have any religious or cultural beliefs. She also denies any medical past history except for having 3 C-sections. She works as an Omnicare Long Term Care pharmacist and orders prescriptions for doctors. She enjoys reading, watching TV, and spending time with her family. -
Aunt
Patient's aunt was born on September 24th, 1981 in Milwaukee. She is a certified pharmacy tech and really enjoys her job. She has 4 children ages 5, 10, 15, and 16. -
Dad
Dad was born February 16th, 1982 and does not work. He is also not involved with the patient. -
Patient
Patient was born in Milwaukee at 27 weeks, 2 pounds 8 ounces. She likes to play Xbox, watch TV, and be with her family. She also enjoys singing and dancing and would like to be a model. She has hydrocephalus and a VP shunt. She has had a total of 6 surgeries, which includes 4 revisions and 2 total shunt replacements. -

Patient NICU
Patient was in the NICU for approximately 9 weeks. She was there due to prematurity and respiratory distress. Mom visited her every day and said that it was a very difficult time. -
Dog
The family got their first pet, a dog named Roxy. -
Sister
Patient's sister is 2 years old and was born in Milwaukee on January 24th, 2011. She has no medical issues. -
Move into apartment
The family moved into a different apartment. They are now 5 minutes away from their other family members and enjoy spending time with them. -
Holiday
The familys' favorite time of the year is the holidays where they spend time talking, eating, and watching TV as a family. -
Disney World
Patient's aunt took her and her cousins to Disney World. She loved the rides and it was her first time in Florida. Her favorite ride was the Tower of Terror. -
Sister
Patient's youngest sister is 10 months old born in Milwaukee on November 13th, 2012. She has no medical issues. -
Shunt Revision
Patient was in the hospital for a VP shunt revision in April 2013. The surgery was successful and she had no complications. -
Shunt Revision
This day is when I had her as my patient. She came in the day before with headaches, lethargy, vomitting, ataxia, and blurry vision. They decided that she would need a revision again because a piece was leaking. This surgery was successful and on this day the patient was recovering. She had some vomitting, but all vitals were stable.
