When Columbia was returnig to the Kennedy Space Center, a small piece of insulating foam broke off the fuel tank. The piece smashed into one wing of the Shuttle and damaged a tile. The tiles protected the Shuttle from heat and when the Columbia returned to Earth, it could not withstand the temperature and it broke apart. All seven crew members died.
