Registering Vital Events

  • Reporting

    All physicians and accouchers in this state must report all births and deaths to the secretary of the board of health in town city or county in which they occur.
  • Birth

    If no physician or accoucher in attendance, then such births or deaths shall be reported by the householder where or under whose observations such birth or death may occur, with the cause of death, if such be known.

    Coroner shall report deaths under his supervision or jurisdiction to City, Town, or County Health Board
  • Marriages

    It shall be the duty of the clerk of the circuit court, of each county to report to the secretary of the county board of health on or before the fifth day of each moth, the number of marriages for the preceding month with such facts relating thereto as may provided for by a blank furnished to such clerk by the secretary of the county board of health
  • Infectious and Contagious diseases must now be reported in additions to Births and deaths:

    If the parents of a newborn child whose birth is unattended is unable to prepare a birth certificate, the LHO shall secure the necessary information from any person having knowledge of the birth and prepare a and file the certificate

    Person in charge of internment shall obtain and enter on the certificate such personal data as my be required by the state board of health, from the persons best qualified to supply such data.

    The person in charge of internment shall present the death to the physician last in attendance upon the deceased, who shall certify the cause of death.

    If death occurred without medical attendance, or the physician last in attendance fails to sign the death certificate, the health officer shall investigate and certify to the cause of death on the basis of information obtained from persons having knowledge of the facts.

    If circumstances suggest that the death was caused by other than natural causes, the health officer shall refer the case to the coroner.

    Births and deaths shall be recorded where the event actually occurred, the place of death in the upper left corner of the standard certificate and the place of actual residence shown in upper right corner.

    " Delayed Certificate of Birth" A certificate of birth presented for filing more than 4 years after the birth shall be deemed a Delayed Certificate of Birth and acceptance shall be subject to the minimum standards prescribed by the SBH, and such records shall be accepted by the SBH

  • Electronic Records

    Beginning 01/01/2011 all births and deaths must be filed in the Indiana Birth registration system (IBRS) and Indiana Death registration system (IDRS)

    Adds that deaths occurring after December 31, 2010 must be entered in IDRS allowing previous paper certificates etc. to be filed on paper so that there would not be a mixture of paper and electronic records for any year.
