Patient born prematurely at 32 weeks. Born with birth defects, developmental delays, PDA (which spontaneously closed), and VSD (R -> L shunting). Patient underwent intestinal surgery with and ileostomy on this day (DOB) as well.

Patient had pull through lap endorectal surgery.

Patient diagnosed with Tethered Cord Syndrome. Had surgery on 1/6/11 to release the tethered cord.

Patient had surgery to repair an imperforate anus, and also had an anorectoplasty.

Patient no longer has an ileostomy (removed).

Patient had a bladder neck plasty surgery.

Patient had CVL placement, a cytoscopy, and a rectal manometry on this day.

Patient had an EGD with biopsies and fluoroscopy on this day.

Patient had an anal exam under anesthesia, a perianal skin biopsy, a cytoscopy, a vaginoscopy, and a proctoscopy. Also had an EGD with biopsies taken on this day.

Patient admitted to hospital for a positive blood culture with CVL. Admitted to West 11.
