Permanent Record and Disclosure of Records

  • A Complete Record

    A complete record must be kept by each board of health on the form prescribed by the state board and shall be open to the inspection of any citizen without fee
  • Record Books

    All records of deaths, births, and cases of contagious and infectious diseases, shall be kept by health officers in record books, the forms of which shall be supplied by the ISBH
  • Records

  • Unlawful Disclosure

    It is unlawful for any officer of employee of the state to disclose the data contain in vital statistical records, except as authorized by this act

    Disclosure of illegitimacy of birth or of information from which it can be ascertained, may be made only upon order of a court.
  • Data Usage

    The ISBH may permit the use of data for research purposes with identifying information
  • We Pay

    Record books must now be furnished at the expense of the city or county.
  • Data Usage

    The LHO may make records of other data for statistical purpose or for planning health programs, the records are not public
  • Permanent Record

    The LHO shall make a permanent record from stillbirth and death certificates.

    1. Name
    2. Sex
    3. Age
    4. Place of death
    5. And residence of deceased
  • Permanent Record

    The LHO shall make a permanent record from each birth certificate containing:
    1. Name
    2. Sex
    3. Date of birth
    4. Place of birth
    5. Names of parents
    6. Birthplaces of parents
    7. File date

    The permanent records shall be open to public inspection

    The LHO shall make a permanent record from stillbirth and death certificates.

    1. Name
    2. Sex
    3. Age
    4. Place of death
    5. Residence of deceased
    6. Voting addresses of the deceased during the 5 years before death
  • 2 Years

    "The LHO shall make a permanent record from stillbirth and death certificates.

    1. Name
    2. Sex
    3. Age
    4. Place of death
    5. Residence of deceased
    6. For a death certificate only, residence addresses of the deceased during the 2 years before death"
  • Copy of Death Certificate

    The LHO shall retain a copy of the certificate of death.
  • Permanent Record

    The LHO shall make a permanent record from each birth certificate containing:
    1. Name
    2. Sex
    3. Date of birth
    4. Place of birth
    5. Names of parents
    6. Birthplaces of parents
    7. File date
    8. The person in attendance at the birth
