  • Mother of patient is pregnant with 1st child

    Mother is not married but biological father is still with her to raise the child.
  • Daughter is born

    1st child is born and is a girl! She is now 11 years old and has ADHD and depression.
  • Mother delivers her 2nd child

    Mother's 2nd child is stillborn following the fetus' diagnosis of a Vein of Galen Aneurysm in Utero. (date and year are not accurate due to sensitivity to the topic, mother did not want to go into detail)
  • Mother delivers her son (My Patient)

    Father was there for the delivery but left the family shortly after. Son reports dad to be a "bad dad"
  • Patient had a left inguinal hernia repair at age 2

    Date is not accurate due to lack of information from mom.
  • Mother is diagnosed with mitral valve prolapse

    Knowledge of this diagnosis tells the nurse that accurate and precise cardiovascular assessment of the patient is extremely important.
  • Patient is diagnosed with eosinophilic esophagitis

    Patient takes fluticasone for treatment of esophagitis, omeprazole for GERD, ondansetron for nausea, and phenol spray for mouth irritation to help treat condition and improve comfort for life.
  • Patient is diagnoed with ADHD

  • Patient is diagnosed with Failure to Thrive

    This diagnosis could be closely connected to the patient's eosinophilic esophagitis as eating can be difficult with this condition.
  • Patient has an EGD Biopsy

  • Patient is admitted to Childrens Hospital West 11

    Patient is admitted to the hospital for vomiting.
  • NG Tube placement

    NG tube is placed in the patient and patient begins to recieve Bright Beginnings in tube for nutrition.
  • Patient is discharged from the hospital

    Mom meets with case manager to establish plan to pay for equipment needed at home as well as Bright Beginnings. Mom also is taught how to use the NG tube, pump, gravity, and proper care for safe return home.
